How to Prepare for Your Nutrition Visit
We want your visit to feel focused, supportive, and productive. A little preparation ahead of time helps us make the most of your session and ensures a smooth experience from start to finish.
Below is everything you need to know before your appointment.
Before Your Appointment
Complete Your Intake Forms
All required intake forms must be completed prior to your visit. These forms allow us to review your health history, goals, and concerns so we can use our time together effectively.
If intake forms are not completed in advance, your appointment may need to be rescheduled.
Verify Your Insurance Coverage
If you plan to use insurance, it is your responsibility to verify your nutrition benefits before your appointment. Coverage varies by plan, diagnosis, and policy.
We recommend confirming:
Coverage for nutrition counseling
Covered diagnoses
Referral requirements (if applicable)
Visit limits per year
Any deductible, copay, or coinsurance
Insurance verification performed by the practice is a courtesy only and does not guarantee coverage or payment.
Ensure Payment Information Is On File
A valid credit card is required to be kept on file for all clients. Any balance identified as patient responsibility by your insurance company will be charged after the claim is processed.
What To Prepare For Your Visit
You do not need to track food or prepare anything complicated unless specifically requested. However, having the following ready can be helpful:
A list of current medications and supplements
Recent lab work (within the last 6–12 months, if available)
Your top 2–3 health goals or concerns
Notes on symptoms, patterns, or challenges you’d like to discuss
Telehealth Appointment Setup
All sessions are conducted via secure, HIPAA-compliant telehealth unless otherwise noted.
To ensure a smooth visit:
Join from a quiet, private space
Use a reliable internet connection
Have your camera enabled
Log in a few minutes early using the appointment link
You must be physically located in a state where the provider is licensed at the time of your appointment.
Insurance & Billing Expectations
Insurance coverage is not guaranteed and varies by plan
When permitted, services may be billed under preventive nutrition counseling benefits
Preventive coverage is not guaranteed and depends on your specific policy
After your visit, your insurance will process the claim and issue an Explanation of Benefits (EOB)
Any balance listed as patient responsibility on the EOB (deductible, copay, or coinsurance) will be charged to the card on file
An EOB is not a bill, but it determines your financial responsibility.
After Your Visit
Following your appointment, you can expect:
A session summary or follow-up plan
Any recommended resources or next steps
Insurance claims to be submitted (if applicable)
Insurance claims may take several weeks to process. Charges related to patient responsibility will occur only after the claim is finalized.
